About This Role Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) who will assess and evaluate members' needs to achieve optimal wellness by guiding them toward appropriate resources. This position is ideal for those who are caring, curious, and committed to improving health outcomes for members. Highlights Location: Remote, IN Schedule: Full-time, Monday through Friday, 8:00 AM to 5:00 PM Work Style: Combination remote work at home and onsite member visits Travel: Must be willing to commute about 70% to meet with members Pay: $71,100 - $97,800 per year Sign-On Bonus: Eligible for a bonus incentive plan What You'll Do Assess and evaluate member needs to facilitate optimal wellness. Identify and resolve barriers to effective care. Develop trusting relationships with members to improve care quality. Coordinate medical and non-medical services for members. Collaborate with the care team to address member needs. Serve as the primary contact for the Interdisciplinary Care Team (ICT). Coordinate care and benefits for members eligible for Medicare. Utilize various strategies to manage members' health issues. Maintain documentation in electronic medical records. Communicate effectively with internal and external stakeholders. Requirements Education: Licensed Registered Nurse (RN) in the state of Indiana without restrictions. Experience: At least 2 years of clinical experience in case management or similar healthcare services. Licenses & Certifications: Current RN License in Indiana.
